The following form helps identify and track the most important criteria for selecting a patent attorney (or patent agent) to work with. Fill out the form as much as possible when considering one or more attorneys to meet with. Then complete the form during and after any meeting. Note that between the USPTO website, the attorney’s website, and resources like Avvo.com, most of the information can be obtained before a meeting.
Do not send the form to an attorney to fill out. While getting the information is important, the process of personally researching the attorney is equally important.
